Local Area Coordination is growing in Swansea and now covers the Cwmtawe area thanks to partnership working from forward-thinking GPs in the Cwmtawe Cluster Network.
The new Local Area Coordinator post is funded by the Welsh Government’s transformation funding which was secured by the Cwmtawe Cluster Network. The Cwmtawe Cluster Network is made up of a group of three GP practices and healthcare professionals, including pharmacists, who work together with the voluntary sector and social services to improve services for patients.
